Billing:  We will  gladly file your insurance for you. 
The patient is responsible for any balances less
co-pays if applicable. Payment is due at the time
of service, unless other arrangements have been
made prior to your visit.

Missed Appointments: Please cancel appointments
at least 24 hours in advance. We understand that
on ocassion emergencies arise and we will take that
into consideration.

Referrals:  If your insurances requires you to have
a  referral or authorizarion to a specialist or for
diagnostic testing, please let us know, so it can be
scheduled in a timely manner.

Pharmacy Refills:  Taking your medication is a very
important aspect of your treatment.  One to two
days prior of your refill date, please have the pharmacy
fax your request.  We make every effort to respond to
all requests on a daily basis. Refill requests will not
be handled over the weekend.

Medical Records:  Having an accurate medical record
is essential to your treatment. Please have your previous
physician forward your records to us or you can fill out a
records release on your first visit. As a courtesy to other
physicians we will copy your medical records free of charge,
however, requests not made by a physician will result in a
medical records fee.
