Upon check-in you will be given a new patient packet and may be  asked to fill out a short questionnaire regarding  any health concerns you may have.  Prepare to arrive at least 15 minutes in advance,  this helps keep the doctor on schedule and   and reduces your wait time.    Please bring your medications, insurance card, identification and  payment.    You will be escorted to an exam room where the your vitals and   medical history will be obtained.    The doctor will exam you and discuss any bloodwork or tests that he  orders. A follow up appointment will be given to discuss the results.
